How do I schedule an appointment?

Click on the “Contact” tab, email or call to schedule an appointment.  Initial appointments include the initial evaluation and treatment.  Please download and complete the patient form to expedite the process.

What do I wear?

Ideally, loose fitting, lightweight shorts and tank top or t-shirt are best.  Sports bras are a good choice to wear for the ladies.

Do I need a prescription?

Most insurance companies require a physician’s prescription and medical diagnosis for reimbursement if you plan on filling your superbill for out of network benefits.  Please request a prescription from your healthcare provider for “PT-eval and treat”.

Why doesn’t Traci Vincent Physical Therapy and Wellness, Inc. bill insurance?

I am a fee for service healthcare practice.  Clients can use their insurance benefits by turning in a superbill receipt for reimbursement using their out-of-network benefits.  As a cash based practice, you will find that my one-on-one, 60 minute sessions will deliver effective results in less time.  Clients often find that out of pocket costs are comparable to co-pays when you add it up.  Insurance rules, billing and documentation requirements are becoming more cumbersome.  Reimbursement rates are decreasing forcing traditional PT clinics to go to a fast paced schedule, reduced treatment times and delegating your care to ancillary personnel in order to see more patients per day.  I am passionate about helping clients find healing and wellness and therefore your care is my priority.  I enjoy connecting with clients to deliver manual therapy that yields results in a non-rushed manner.  It is through the cash based business model that I can provide this level of care.

What are the fees?

The fee for initial evaluation and treatment is $160.

All follow-up treatments are $135.

Fitness training and yoga sessions are $50

Wellness and business coaching are free. Products are extra.
